At your first appointment you will be required to complete a client intake form, which outlines your basic details (name, address etc.) and includes some general questions about your past and current health. You will also be required to sign a consent form which indicates that you have received a sufficient explanation regarding your treatment and that you now consent to having the treatment. All the information you provide is confidential and your written consent will be required for any release of your information to other health care or insurance providers.
Ocean Breeze is easily accessible via public transit. Take the #20 to Herring Cove (you can transfer at the Mumford Terminal if needed). Once you see the large green sign for Herring Cove, Sambro, and Purcells Cove, request the next stop. Ocean Breeze is a just a few steps past the bus stop. To return to Halifax, the outbound bus stop is just past the mailboxes across the road. It takes approximately a half hour to travel from the Mumford Terminal to the Centre.
